Each Counselor has their own appointment request forms located at Kim's desk, fill out the form and drop it in the black tray.
The slips are delivered to the counselors every class period.

If it is an URGENT SAFETY Concern, give the request sheet to Kim or tell a Counseling Aide
that you need a counselor now.

It is helpful to your Counselor or College and Career Advisor to be as specific as possible on your request sheet. If it is a personal matter that you do not want to specify on the sheet that is completely acceptable.
